You received a link to a form.
A clinic that uses Readily Health has asked you to answer a short questionnaire about a patient. This page explains what happens next, how to check the email is genuine, and what to do if the link doesn't work.

If you were not expecting this message, you can ignore it.
No patient name, no health details, no questionnaire name.
- The clinic's name, as you know it.
- Nothing about the patient. That's on purpose, so it is fine that the email says very little.
- The link starts with https://app.readilyhealth.com/
What you'll need.
Parent or guardian
- The child's date of birth
- A debit or credit card, if the clinic charges a fee
If a teacher is also asked to help, you'll see their access code when you finish. Pass it on in person or by phone.
Teacher
- The access code from the child's parent or guardian. It is not in the email.
You will not be asked to pay anything.
Adult patient
- Your date of birth
- A debit or credit card, if the clinic charges a fee
Your answers go to your clinician, who will go over them with you.
What happens when you open it.
Confirm your details
Enter the patient's date of birth, or the access code if you're a teacher. This keeps the form private if the email reaches the wrong person.
Read how your answers are used
A short note from the clinic explains who sees your answers and what they are for.
Pay, if the clinic asks
The fee is paid on a secure page run by our payment partner, Stripe, and you come straight back. Teachers skip this step.
Answer the questions
Most people finish in about 10 minutes. Answers save as you go, so you can stop and open the same link later to continue.
Send your answers
They go to the clinic. You won't see a score on the website, and the link can't be used again.
Is the email genuine?
-
The link starts with https://app.readilyhealth.com/ and nothing else in front of it.
-
Readily Health never asks for a date of birth, card number or answers by email, text or phone. You only enter them inside the form, after you open the link.
-
Payment happens on a page at checkout.stripe.com, opened from inside the form.
-
Still unsure? Call the clinic on a number you already have, not one from the email, and ask whether they sent it.
If something goes wrong.
The clinic that sent the link can resend it, give you more time or cancel it. Readily Health can't look up a form for you.
It says the link is no longer valid
Links expire after a set time, stop working once the form is sent, and fail if part of the address was cut off. Open it straight from the email rather than copying it. If that doesn't help, ask the clinic for a new link: the new one replaces the old one.
The link was locked after wrong attempts
After five details that don't match, the link locks to protect the patient. Ask the clinic for a new link. Parents: use the child's date of birth, not your own.
I'm a teacher and don't have an access code
Ask the child's parent or guardian: they receive it when they finish their part. If they can't find it, the clinic can send you a new link with a new code.
I paid, but the page is still confirming
Confirmation can take a minute. Wait a moment, then choose Check again. You will not be charged twice.
I need to stop halfway
Your answers are saved as you go. Open the same link from the email when you're ready. If you've been away a while, you'll confirm your details again and pick up where you left off.
I made a mistake after sending
The link can't be reopened once your answers are sent. Tell the clinic: they can note the correction or send a new form.
I have a question about the fee or a refund
Fees are set and refunded by the clinic. Contact them directly.
Your answers go to the clinic.
Only the clinic that sent the link sees what you wrote. Its clinicians review your answers and talk them through with you or the patient. This is a screening questionnaire: it helps the clinician decide what to look at next, and it is not a diagnosis.
The browser tab only ever says "Secure form", so nothing on a shared screen gives away what the form is about.
Who to contact.
- Questions about the form, the appointment or the fee
- The clinic that sent you the link.
- An email that looks like it's from us but doesn't seem right
- Forward it to contact@readilyhealth.com. Please don't add the patient's name or any health details.
